Hernandez v. Meadows is a case that was filed by advocates, including Florida Legal Services, Legal Aid of Palm Beach County, and the National Health Law Program against the Florida Medicaid Agency for failure to provide written notice and fair hearings to beneficiaries when prescription drugs were denied. The parties agreed to settle the case.

The materials set forth below were developed by Plaintiffs and Defendants to implement the settlement agreement. We are posting these materials so that advocates in other states can use them as an example in addressing notice and hearing issues in their Medicaid prescription drug programs.
